The ꜂ standing outside the opening bracket is a newcomer: no kaomoji on this site used it before this one. Its Unicode name is MODIFIER LETTER CHINESE TONE YIN SHANG, while the ꜄ that closes the face is a different letter, YIN QU, and both come from a set of marks written at the corners of a Chinese character to show its tone, the shang mark at the top left and the qu mark at the top right, which are exactly the ends they occupy here. A second ꜄ has slipped inside the bracket after three blank spaces, right in front of eyes made of two full stops and an ω mouth. Whoever posted it called it a cat that is "ochiteru", the casual form of ochite iru, which describes something that has fallen and is still lying where it landed; you could say it of a wallet on the pavement, and here it is used, playfully, for a cat flopped flat on the floor.
